What are Thrombolytics?
- Alteplase is an enzyme that occurs naturally in man and causes blood clots to dissolve. It is a man-made protein manufactured by recombinant DNA technology. The naturally occurring protein, known as tissue plasminogen activator (TPA), is made by ovarian cells from the Chinese hamster.
- Standard Treatment: tPA to Dissolve the Blood Clot. The technique for treating blood clots causing stroke is called intravenous thrombolysis—in lay terms, “clot-busting.” Thrombolysis is the combination of two Greek words, “thrombus,” meaning blood clot, and “lysis,” meaning to dissolve or break apart.
- Experts said the findings could open up the treatment option for many more stroke victims. The drug is called tissue plasminogen activator, or tPA. It can dissolve the blood clot causing a person's stroke, and protect brain tissue from any further damage. However, tPA has to be given within a certain time frame to work.
Thrombolytics are medicines that may be used for the emergency treatment of an ischemic stroke (a stroke caused by a blood clot), a heart attack (myocardial infarction), or a massive pulmonary embolism (PE). They may also be used for other indications.
Thrombolytics break up clots by activating fibrinolysis and converting plasminogen to plasmin which lyses the clot. This allows blood and oxygen to reperfuse the area, limiting tissue damage. Thrombolytics are more effective if initiated as soon as possible after the event (such as a heart attack, stroke, or PE) but can be given within three hours of symptom onset for a stroke, or within 12-24 hours of symptom onset for a heart attack.
Major bleeding, including bleeding into the brain, is the most common risk associated with thrombolytics.
